Sound from both speakers and headphones?

Hello, I've been having a problem lately with the sound of my computer. As the title says, sound is comming from both my speakers and my headphones. Yes, they are connected all the way in and I have uninstalled and reinstalled my sound card software twice now and the problem hasn't changed. Does anyone thing that it might be something wrong with my hardware?

with your headphones unplugged, go to control panel>hardware and sound>connection settings>click the box to allow pop-ups when a device is inserted then click ok. plug in your headphones and a box should pop up then click on headphones.
