Hey there, nil.
You could try accessing the USB flash drive via Ubuntu Live USB/DVD and try to reformat it, to see if that fixes the issue. Note that this will delete the data from the pen drive. Other than that you could try the options you've already tried, with another computer, to see if you get the same results. Unfortunately, if nothing seems to help, the USB flash drive might have already failed. It's not a rare case that when a flash drive fails it locks itself and becomes unusable.
