To create ads on Instagram, your account must be a business account. If this is not the case, you'll need to convert it. You must also be the admin of the Facebook page that's linked to the Instagram account.
If your account meets the requirements, then you can try to make appeal. As it seems you're using cellular data, try using WiFi instead to access the form. Alternatively, try using the Instagram website.
