Monitor & Freeze, but CPU still running

Hello,
I am having some problem with my PC. Everytime I switched on and after using for about 5 minutes, my monitor will be turn black or sometimes freezes. At the same time, the mouse also freezes and there is no light on my mouse(indicating that is not working). But when these happen, my CPU is still running. After restarting my CPU, everything becomes normal again. Can anyone tell me is this a software or hardware problem?

I suspect a hardware problem since the light goes off on the mouse. There are multiple voltages that the power supply sends to the motherboard (MB). It could be either a MB problem or a power supply problem. I repair should should be able to diagnose the problem.
