The most certain cause is that the account was deactivated or deleted.

Thank you for this but why would someone follow me then immediately ( within 5 mins ) deactivate their account

It's impossible to tell you exactly why - there can be many reasons; the account was used for testing purposes, deleted by Instagram because it was a fake account, the person prefer to use an account privately but realized that it was public when using it, or simply because they changed their mind about having an account...
